Infertility is the inability of a couple to conceive despite one full year of unprotected intercourse. The most common cause of infertility is male factors, including primary testicular disease (30% to 40%), disorders of sperm transport (10% to 20%), and hypothalamic pituitary disease (1% or 2%). Ovulatory dysfunction is the second most common cause of infertility (18%). Tubal and peritoneal factors make up the third most common factor in infertility (22%), with pelvic inflammatory disease and subclinical pelvic chlamydia infections as major contributors. Infertile women may also have endometriosis at a high prevalence rate. Unexplained factors make up about 28% of infertility causes. These couples may benefit from empiric therapy of ovulatory stimulation with clomiphene citrate and/or intrauterine insemination. The cervical factor is considered the midcycle interaction of spermatozoa and cervical mucus, and it is not as common in infertility as the factors listed previously. The uterine factor is generally associated with recurrent pregnancy loss and is not generally considered a cause of infertility. However, myomas may cause infertility if they are located near the uterotubal junction or are large enough to distort the anatomy of the fallopian tube and ovary. A complete sexual history is essential in assessing infertility at the initial visit.
